Aims and Objectives
Health Vault aims to deliver trauma aware care that is of high-quality, community-based support for children and adults with mental health needs that maybe complex ,including those with dual diagnosis, where mental health is the primary diagnosis and autism, learning disability, or behaviour that challenges may be present as secondary needs. Our purpose is to provide proactive, person-centred care that supports stability, promotes independence, and prevents avoidable hospital admissions
